At approximately 8:40am yesterday, emergency services were called to Forest Rd in Hurstville after receiving reports of a single-vehicle crash.
A police spokesperson told 7NEWS.com.au that a car had rolled on to the boy, trapping him underneath.
Paramedics freed the boy and treated him at the scene before taking him to Sydney Children's Hosptial at Randwick in a critical condition.
The driver (identified as a 36-year-old male) was arrested at the scene and taken to hospital for mandatory testing.
According to NSW Ambulance Inspector David Stride, bystanders, including an off-duty nurse, tried to resuscitate the boy while waiting for ambulance crews to arrive.
"Unfortunately the child was unable to be revived," said Inspector Stride, noting that five ambulance crews and a medical team were sent to the scene.
Stride said it was a "very distressing" for everyone involved.
"There were a lot of people upset, a lot of people who witnessed the accident."
