The actress was listed on the application under her married name Katherine Leigh Webb, which was lodged by Constable Dean Hodges.
Under the AVO, Webb cannot assault, or attempt or threaten to assault Katherine Leigh Ritchie; destroy or damage her property (or threaten to do this); or stalk, harass or intimidate her.
The former NRL player is also banned from approaching his wife within 12 hours of consuming alcohol or taking illicit drugs.
During the court hearing, Webb sat in silence, other than to indicate his presence and to plead guilty to the offence of mid-range drunk driving.
Kate has allegedly moved out of their home and is living away from Stuart but the pair were mentioned in court as being still married.
Webb did not argue against the provisions mentioned and was silent in court.
Webb was also charged with mid-range drink driving and pleaded guilty to the offence, before being sentenced to a 12 months community corrections order and five months disqualified from driving, already served.
